Here's an example of what I would write and what you should follow (you don't have to enter the things in brackets):

(Favorite Faction[s]) - European Enforcer Corps
(Favorite Map[s]) - Three Mile Island, Rovaniemi Air Base, U.S.S. Reagan Sea Base
(Favorite Battle Type[s]) - Conquest, Battle
(Favorite Unit[s]) - Tank, Artillery, Command Vehicle
(Favorite Personal Style) - Establish front line with tanks, bring in artillery, and slowly advance
(Prefered Command View) - SitRep
(Prefered Gameplay) - Voice Command
(Any other Notes can be placed here)
